CLINIC FEES

When paid by cash or check:

Adult clients 65 and older are eligible for a discount of 15%,

Adult clients under 65 are eligible for a discount of 10% when;

• Paid in full at time of visit
• No prior balance outstanding

When paid by debit/credit card:

Adult clients 65 and older are eligible for a discount of 10%,

Adult clients 65 and under are eligible for a discount of 5% when;

• Paid in full at time of visit
• No prior balance outstanding

No Discount is available when only purchasing supplements/supplies; No Discount is available on visits or supplements when insurance is billed for visits

“Accepting a discount is a binding private pay agreement. You will not submit to insurance and we will not provide ANY INSURANCE CODES.”

• A $50.00 fee will be charged for cancellations with less than 24 hours notice.
